El NEAT és on viu la major part de la veritable variació entre persones — moure's, postura, moviment espontani. També és el component que cau silenciosament durant una dieta, que és una gran part del que les persones experimenten com el seu metabolisme "lent".

Aigua doblement marcada

El mètode de referència funciona fent-te beure aigua marcada amb isòtops estables d'hidrogen i oxigen, i després rastrejant la seva eliminació diferencial. La diferència entre ells reflecteix la producció de diòxid de carboni, i per tant el consum energètic, durant una a dues setmanes de vida ordinària. És car i és indiscutible.

Una revisió sistemàtica del consum energètic total mesurat d'aquesta manera en adults dóna la imatge actual [2], i les anàlisis agrupades han produït estimacions tant del consum en repòs com del total a través de les poblacions [4][5]. La feina amb nens amb malalties cròniques mostra la mateixa metodologia aplicada on les equacions de predicció fallen més [1].

Una meta-anàlisi va validar els mètodes d'avaluació dietètica contra l'aigua doblement marcada [3]. La troballa és incòmoda i consistent: les persones subreporten substancialment la ingesta, i la subreportació és més gran en persones amb un pes corporal més alt. No és un engany deliberat — l'estimació de porcions és realment difícil, i cap diari alimentari captura l'oli de cuina amb precisió.

Pel que fa al costat del consum, les equacions predictives es van comparar sistemàticament en adults sans i en atletes, i existeixen orientacions de bones pràctiques per mesurar adequadament la taxa metabòlica en repòs. Totes són mitjanes poblacionals amb barres d'error individuals de diversos centenars de quilocalories.

Disponibilitat energètica, no només ingesta

Un concepte distint és important per a qualsevol que entri en un entrenament seriós: la disponibilitat energètica és la ingesta menys el consum d'exercici, en relació amb la massa lliure de greix. Una disponibilitat crònicament baixa suprimeix la funció fisiològica molt abans que sembli un trastorn alimentari. Un assaig aleatoritzat va trobar que una baixa disponibilitat energètica reduïa la síntesi de proteïnes miofibril·lars i sarcoplasmàtiques, i s'han provat intervencions d'educació nutricional per abordar-ho.

El son interactua amb tot això — la restricció del son va canviar la composició de la pèrdua de pes en un assaig aleatoritzat, i l'extensió del son va canviar la ingesta energètica mesurada objectivament [6]. Més informació al nostre lloc d'evidència sobre el son.

Canvia la composició de la dieta el consum?

Una mica, i la magnitud de l'efecte és realment discutida. Un assaig aleatoritzat prominent va examinar el consum energètic durant el manteniment de la pèrdua de pes en una dieta baixa en carbohidrats [7] i va informar d'una diferència; l'anàlisi ha estat objecte de debat extensament des de llavors. El resum honest és que qualsevol efecte és petit en relació amb la diferència entre el que les persones pensen que mengen i el que realment mengen.

Preguntes freqüents

Tinc el metabolisme lent?
Gairebé segur que no, almenys no en un grau que expliqui un problema de pes. Ajustada per la composició corporal, la taxa metabòlica basal varia molt menys entre persones del que se sol suposar — i l'error de mesura en el seguiment de la ingesta és molt més gran.
Val la pena fer-se una prova metabòlica en un gimnàs?
La calorimetria indirecta pot ser precisa si es fa en condicions adequades — en dejú, en repòs, correctament calibrada. La majoria de les proves comercials no compleixen aquestes condicions.
Menjar més sovint accelera el metabolisme?
No. L'efecte tèrmic dels aliments s'escala amb la quantitat que menges, no amb el nombre d'àpats en què la divideixes.
Per què s'ha estancat la meva pèrdua de pes?
