Stofskiftet er en af de mest målbare ting ved dig, og en af de mest konsekvent fejlbeskrevne.

NEAT er der, hvor det meste af den reelle variation mellem personer findes — rastløs bevægelse, kropsholdning, spontane bevægelser. Det er også den komponent, der falder stille og roligt under en diæt, hvilket er en stor del af det, folk oplever som, at deres stofskifte "sætter ned".
Dobbeltmærket vand
Referencemetoden fungerer ved, at man drikker vand mærket med stabile isotoper af hydrogen og oxygen, hvorefter man følger deres forskellige udskillelse. Forskellen mellem dem afspejler kuldioxidproduktionen og dermed energiforbruget over en til to ugers almindeligt liv. Metoden er dyr, og den er uomtvistelig.
Et systematisk review af det totale energiforbrug målt på denne måde hos voksne giver det aktuelle billede [2], og poolede analyser har frembragt estimater af både hvile- og totalforbrug tværs af populationer [4][5]. Studier af børn med kronisk sygdom viser samme metodologi anvendt, hvor prædiktive ligninger slår mest fejl [1].
Dobbeltmærket vand som referencemetode

En meta-analyse validerede metoder til kostregistrering mod dobbeltmærket vand [3]. Resultatet er ubehageligt og konsistent: folk underrapporterer deres indtag betydeligt, og underrapporteringen er større hos personer med højere kropsvægt. Det er ikke bevidst bedrag — portionsestimering er reelt svært, og ingen kostdagbog registrerer stegeolie nøjagtigt.
På forbrugssiden er prædiktive ligninger blevet sammenlignet systematisk hos raske voksne og hos atleter, og der findes best-practice-vejledning til korrekt måling af hvilestofskiftet. Alle er populationsgennemsnit med individuelle fejlmargener på nogle hundrede kilokalorier.
Energitilgængelighed, ikke kun indtag
Et separat begreb er vigtigt for alle, der træner seriøst: energitilgængelighed er indtag minus energiforbrug ved træning, i forhold til fedtfri masse. Kronisk lav tilgængelighed hæmmer fysiologisk funktion, længe før det ligner en ædeforstyrrelse. Et randomiseret studie fandt, at lav energitilgængelighed reducerede den myofibrillære og sarkoplasmatiske proteinsyntese, og der er afprøvet ernæringsundervisningsindsatser for at afhjælpe det.
Søvn spiller sammen med alt dette — søvnrestriktion ændrede sammensætningen af vægttabet i et randomiseret studie, og forlænget søvn ændrede det objektivt målte energiindtag [6]. Læs mere på vores søvnvidenssite.
Ændrer kostsammensætningen energiforbruget?
En smule, og størrelsen af effekten er virkelig omdiskuteret. Et fremtrædende randomiseret studie undersøgte energiforbruget under vægttabsbevarelse på en lavkulhydratdiæt [7] og rapporterede en forskel; analysen er siden blevet diskuteret intensivt. Den ærlige opsummering er, at en eventuel effekt er lille i forhold til forskellen mellem, hvad folk tror, de spiser, og hvad de faktisk spiser.
