NEAT on se, missä suurin osa aidosta ihmisten välisestä vaihtelusta piilee — hypistely, ryhti, spontaani liike. Se on myös osatekijä, joka vähenee huomaamatta laihdutuksen aikana, mikä on suuri osa siitä, mitä ihmiset kokevat aineenvaihduntansa "hidastumisena".
Kaksoismerkitty vesi
Vertailumenetelmässä juodaan vetyn ja hapen pysyvillä isotoopeilla merkittyä vettä, jonka jälkeen seurataan niiden erilaista poistumisnopeutta. Isotooppien poistumisnopeuksien välinen ero kuvastaa hiilidioksidin tuotantoa ja siten energiankulutusta yhden tai kahden viikon tavallisen elämän aikana. Menetelmä on kallis, mutta kiistaton.
Systemaattinen katsaus, jossa aikuisten kokonaisenergiankulutusta on mitattu tällä menetelmällä, antaa nykyisen kokonaiskuvan [2], ja yhdistetyt analyysit ovat tuottaneet arvioita lepo- ja kokonaisenergiankulutuksesta eri väestöissä [4][5]. Kroonisia sairauksia sairastavilla lapsilla tehty tutkimus osoittaa samaa menetelmää sovellettuna tilanteissa, joissa ennusteyhtälöt epäonnistuvat pahimmin [1].
Kaksoismerkitty vesi vertailumenetelmänä
Miksi kuntorannekkeesi ja sovelluksesi ovat eri mieltä
Meta-analyysi validoi ruokavalion arviointimenetelmiä kaksoismerkittyä vettä vasten [3]. Havainto on epämukava ja johdonmukainen: ihmiset aliraportoivat saantiaan huomattavasti, ja aliraportointi on suurempaa suuremman ruumiinpainon omaavilla ihmisillä. Kyse ei ole tahallisesta harhaanjohtamisesta — annoskokojen arviointi on aidosti vaikeaa, ja mikään ruokapäiväkirja ei tallenna ruokaöljyn määrää tarkasti.
Kulutuksen puolella ennusteyhtälöitä on verrattu systemaattisesti terveillä aikuisilla ja urheilijoilla, ja lepoaineenvaihdunnan asianmukaiseen mittaamiseen on olemassa parhaiden käytäntöjen ohjeistus. Kaikki ne ovat väestökeskiarvoja, joiden yksilölliset virhemarginaalit ovat muutaman sata kilokaloria.
Energian saatavuus, ei vain saanti
Vakavasti harjoittelevalle on tärkeä erillinen käsite: energian saatavuus on saanti miinus liikunnan kulutus, suhteutettuna rasvattomaan massaan. Krooninen alhainen saatavuus heikentää fysiologista toimintaa kauan ennen kuin se muistuttaa syömishäiriötä. Satunnaistetussa tutkimuksessa havaittiin, että alhainen energian saatavuus vähensi myofibrillaarista ja sarkoplasmista proteiinisynteesiä, ja asian korjaamiseksi on kokeiltu ravitsemuskasvatusinterventioita.

Muuttaako ruokavalion koostumus energiankulutusta?
Vähän, ja vaikutuksen suuruudesta kiistellään aidosti. Merkittävä satunnaistettu tutkimus tarkasteli energiankulutusta painonpudotuksen ylläpitovaiheessa vähähiilihydraattisella ruokavaliolla [7] ja raportoi eron; analyysista on kiistelty laajasti sen jälkeen. Rehellinen yhteenveto on, että mahdollinen vaikutus on pieni verrattuna erotukseen sen välillä, mitä ihmiset luulevat syövänsä ja mitä he todella syövät.
