Como é medida a despesa energética humana
A taxa metabólica é uma das coisas mais mensuráveis sobre você e uma das mais frequentemente mal descritas.

O NEAT é onde a maior parte da genuína variação entre pessoas reside — inquietação, postura, movimento espontâneo. É também o componente que cai silenciosamente durante uma dieta, que é uma grande parte do que as pessoas experienciam como seu metabolismo "diminuindo".
Água duplamente marcada
O método de referência funciona fazendo você beber água marcada com isótopos estáveis de hidrogênio e oxigênio, e depois rastreando sua eliminação diferencial. A diferença entre eles reflete a produção de dióxido de carbono e, portanto, a despesa energética, ao longo de uma a duas semanas de vida ordinária. É caro e é inegável.
Uma revisão sistemática da despesa energética total medida dessa forma em adultos fornece a imagem atual [2], e análises agrupadas produziram estimativas tanto da despesa de repouso quanto total entre populações [4][5]. Trabalhos em crianças com doenças crônicas mostram a mesma metodologia aplicada onde as equações preditivas falham com mais força [1].
Água duplamente marcada como método de referência

Uma meta-análise validou métodos de avaliação dietética contra água duplamente marcada [3]. A descoberta é desconfortável e consistente: as pessoas sub-relatam substancialmente a ingestão, e o sub-relato é maior em pessoas com maior peso corporal. Não é uma enganação deliberada — a estimativa de porções é genuinamente difícil, e nenhum diário alimentar captura o óleo de cozinha com precisão.
Do lado da despesa, as equações preditivas foram comparadas sistematicamente em adultos saudáveis e em atletas, e existem orientações de melhores práticas para medir corretamente a taxa metabólica de repouso. Todas elas são médias populacionais com barras de erro individuais de algumas centenas de quilocalorias.
Disponibilidade de energia, não apenas ingestão
Um conceito distinto é importante para quem treina seriamente: a disponibilidade de energia é a ingestão menos a despesa de exercício, relativa à massa livre de gordura. A disponibilidade cronicamente baixa suprime a função fisiológica muito antes de parecer um transtorno alimentar. Um ensaio randomizado descobriu que a baixa disponibilidade de energia reduzia a síntese de proteínas miofibrilares e sarcoplasmáticas, e intervenções de educação nutricional foram testadas para abordá-la.
O sono interage com tudo isso — a restrição do sono alterou a composição da perda de peso em um ensaio randomizado, e a extensão do sono alterou a ingestão de energia medida objetivamente [6]. Mais em nosso site de evidências sobre sono.
A composição da dieta altera a despesa?
Um pouco, e o tamanho do efeito é genuinamente contestado. Um ensaio randomizado proeminente examinou a despesa energética durante a manutenção da perda de peso em uma dieta baixa em carboidratos [7] e relatou uma diferença; a análise tem sido amplamente debatida desde então. O resumo honesto é que qualquer efeito é pequeno em relação à diferença entre o que as pessoas pensam que comem e o que realmente comem.
