Resistência à insulina, mecanicamente
A resistência à insulina é a lesão central no diabetes tipo 2 e um dos conceitos mais frequentemente mal explicados na nutrição.
O que é realmente resistente
A insulina instrui os músculos e a gordura a absorver glicose e diz ao fígado para parar de produzi-la. Na resistência à insulina, essas instruções são menos eficazes, então o pâncreas compensa produzindo mais. A glicose no sangue permanece normal por anos enquanto a insulina aumenta — o que é a razão pela qual a condição geralmente está bem avançada antes que um teste de glicose de rotina a detecte. O diabetes tipo 2 aparece quando a compensação falha.

Onde a explicação popular está errada
A história comum é: os carboidratos aumentam a insulina, a insulina cronicamente alta causa resistência, portanto, os carboidratos causam resistência à insulina. Cada passo contém algo verdadeiro, e a conclusão não se segue.
- Populações que consomem dietas tradicionais com muito carboidrato historicamente tiveram baixas taxas de resistência à insulina. A relação não é uma simples resposta à dose com carboidratos dietéticos.
- A gordura ectópica é mais preditiva do que a ingestão de carboidratos. Lipídios acumulando dentro das células do fígado e dos músculos interferem diretamente na sinalização da insulina.
- O excesso de energia é o fator comum a montante. O sobrepeso produz resistência à insulina independentemente do macronutriente que fornece o excesso.
- A inatividade física é causalmente independente. Músculos que não estão sendo usados eliminam glicose de forma inadequada, razão pela qual o repouso na cama induz resistência à insulina mensurável em poucos dias.

A meta-análise do treinamento aeróbico combinado com treinamento de resistência sobre o controle glicêmico [2] merece destaque porque o efeito é parcialmente independente da mudança de peso — o exercício melhora a eliminação de glicose por mecanismos que não exigem que você fique menor.

A resistência à insulina está subjacente a várias condições que muitas vezes são discutidas separadamente. A disfunção do tecido adiposo na síndrome dos ovários policísticos foi revisada sistematicamente [3], assim como o papel da modificação do estilo de vida na SOP [6]. O diabetes gestacional compartilha muito da mesma fisiopatologia [1]. E o papel do microbioma intestinal no diabetes tipo 2 foi revisado [7], uma área com fortes associações e fraca evidência causal em humanos.
