Insulinresistenz, mechanistisch
Insulinresistenz ist die zentrale Läsion bei Typ-2-Diabetes und eines der am häufigsten missverstandenen Konzepte in der Ernährung.
Was tatsächlich resistent ist
Insulin weist Muskeln und Fett an, Glukose aufzunehmen, und sagt der Leber, die Produktion zu stoppen. Bei Insulinresistenz sind diese Anweisungen weniger effektiv, sodass die Bauchspeicheldrüse kompensiert, indem sie mehr produziert. Der Blutzucker bleibt jahrelang normal, während Insulin steigt — weshalb der Zustand typischerweise weit fortgeschritten ist, bevor ein routinemäßiger Glukosetest ihn erkennt. Typ-2-Diabetes tritt auf, wenn die Kompensation versagt.

Wo die populäre Erklärung falsch liegt
Die gängige Geschichte lautet: Kohlenhydrate erhöhen Insulin, chronisch hohes Insulin verursacht Resistenz, daher verursachen Kohlenhydrate Insulinresistenz. Jeder Schritt enthält etwas Wahres, und die Schlussfolgerung folgt nicht.
- Bevölkerungen, die sehr kohlenhydratreiche traditionelle Diäten essen, hatten historisch niedrige Raten von Insulinresistenz. Die Beziehung ist keine einfache Dosis-Wirkungs-Beziehung mit diätetischen Kohlenhydraten.
- Ektopisches Fett ist prädiktiver als die Kohlenhydrataufnahme. Lipide, die sich in Leber- und Muskelzellen ansammeln, stören direkt die Insulin-Signalübertragung.
- Energieüberschuss ist der gemeinsame übergeordnete Faktor. Überernährung führt unabhängig von dem Makronährstoff, der den Überschuss liefert, zu Insulinresistenz.
- Körperliche Inaktivität ist unabhängig kausal. Unbenutzte Muskeln verarbeiten Glukose schlecht, weshalb Bettruhe innerhalb weniger Tage messbare Insulinresistenz induziert.

Die Meta-Analyse über kombiniertes aerobes und Widerstandstraining zur glykämischen Kontrolle [2] ist besonders hervorzuheben, da der Effekt teilweise unabhängig von Gewichtsveränderungen ist — Bewegung verbessert die Glukoseverwertung durch Mechanismen, die nicht erfordern, dass man kleiner wird.

Insulinresistenz liegt mehreren Erkrankungen zugrunde, die oft separat diskutiert werden. Die Dysfunktion des Fettgewebes bei polyzystischem Ovarialsyndrom wurde systematisch überprüft [3], ebenso die Rolle von Lebensstiländerungen bei PCOS [6]. Gestationsdiabetes teilt sich viel von der gleichen Pathophysiologie [1]. Und die Rolle des Mikrobioms des Darms bei Typ-2-Diabetes wurde überprüft [7], ein Bereich mit starken Assoziationen und schwachen kausalen Beweisen beim Menschen.
Kräuterheilmittel für PCOS wurden ebenfalls systematisch überprüft [5] — hier hauptsächlich enthalten, weil die Überprüfung eine nützliche Demonstration dafür ist, wie dünn diese Literatur ist.
