NEAT je tisto, kjer se nahaja večina resnične variabilnosti med osebami — nemirjenje, drža, spontano gibanje. To je tudi komponenta, ki tiho upada med dieto, kar je velik del tega, kar ljudje doživljajo kot "upadanje" njihovega metabolizma.

Dvojno označena voda

Referenčna metoda deluje tako, da spijete vodo, označeno s stabilnimi izotopi vodika in kisika, nato pa spremljate njuno diferencialno izločanje. Razlika med njima odraža proizvodnjo ogljikovega dioksida in torej porabo energije, v obdobju enega do dveh tednov običajnega življenja. Je draga in ni sporna.

Sistematični pregled skupne porabe energije, izmerjene na ta način pri odraslih, daje trenutni vpogled [2], in združene analize so proizvedle ocene tako resting kot skupne porabe med populacijami [4][5]. Delo z otroki s kroničnimi boleznimi kaže enako metodologijo, ki se uporablja tam, kjer napovedne enačbe najbolj odpovejo [1].

Meta-analiza je potrdila metode ocenjevanja prehrane v primerjavi z dvojno označeno vodo [3]. Ugotovitev je neprijetna in dosledna: ljudje znatno podcenjujejo vnos, podcenjevanje pa je večje pri ljudeh z višjo telesno težo. To ni namerna zavajanje — ocenjevanje porcij je resnično težko, in noben prehranski dnevnik ne zajame kuhinjskega olja natančno.

Na strani porabe so bile napovedne enačbe sistematično primerjane pri zdravih odraslih in športnikih, in obstajajo smernice za najboljšo prakso za pravilno merjenje resting metabolic rate. Vse so povprečja populacije z individualnimi napakami v nekaj sto kilokalorijah.

Razpoložljivost energije, ne le vnos

Pomemben koncept je pomemben za vsakogar, ki resno trenira: razpoložljivost energije je vnos minus poraba pri vadbi, v sorazmerju z brezmaščobno maso. Kronično nizka razpoložljivost potlači fiziološko funkcijo še preden izgleda kot motnja hranjenja. Randomizirana študija je pokazala, da nizka razpoložljivost energije zmanjšuje sintezo miofibrilarnih in sarkoplazemskih beljakovin, in intervencije izobraževanja o prehrani so bile preizkušene za reševanje tega problema.

Ali sestava diete spremeni porabo?

Malce, in velikost učinka je resnično sporna. Izstopajoča randomizirana študija je preučila porabo energije med vzdrževanjem teže po dieti z nizko vsebnostjo ogljikovih hidratov [7] in poročala o razliki; analiza je bila obsežno razpravljana od takrat. Pošten povzetek je, da je vsak učinek majhen v primerjavi z razliko med tem, kar ljudje mislijo, da jedo, in tem, kar dejansko jedo.

Ali imam počasen metabolizem?
Skoraj zagotovo ne v meri, ki bi pojasnila težavo s telesno težo. Prilagojena telesni sestavi, bazalna presnova med ljudmi variira veliko manj, kot se domneva — napaka pri merjenju vnosa hrane pa je veliko večja.
Ali je metabolični test v fitnesu vreden svoje cene?
Indirektna kalorimetrija je lahko natančna, če je izvedena pod ustreznimi pogoji — na tešče, spočito, pravilno umerjeno. Večina komercialnega testiranja teh pogojev ne izpolnjuje.
Ali pogostejši obroki pospešijo metabolizem?
Ne. Termični učinek hrane je odvisen od tega, koliko pojeste, ne od tega, na koliko obrokov to razdelite.
Zakaj se je moje hujšanje ustavilo?
