Cómo se mide el gasto energético humano
La tasa metabólica es una de las cosas más medibles sobre ti y una de las más consistentemente mal descritas.

NEAT es donde vive la mayor parte de la genuina variación entre personas: moverse inquietamente, postura, movimiento espontáneo. También es el componente que disminuye silenciosamente durante una dieta, que es una gran parte de lo que las personas experimentan como su metabolismo "ralentizándose".
Agua doblemente etiquetada
El método de referencia funciona haciendo que bebas agua etiquetada con isótopos estables de hidrógeno y oxígeno, luego rastreando su eliminación diferencial. La diferencia entre ellos refleja la producción de dióxido de carbono y, por lo tanto, el gasto energético, durante una o dos semanas de vida ordinaria. Es caro y es indiscutible.
Una revisión sistemática del gasto energético total medido de esta manera en adultos da la imagen actual [2], y los análisis agrupados han producido estimaciones tanto del gasto en reposo como del total en diferentes poblaciones [4][5]. El trabajo en niños con enfermedades crónicas muestra la misma metodología aplicada donde las ecuaciones de predicción fallan con más fuerza [1].
Agua doblemente etiquetada como método de referencia

Un metaanálisis validó los métodos de evaluación dietética contra agua doblemente etiquetada [3]. El hallazgo es incómodo y consistente: las personas subestiman sustancialmente la ingesta, y el subregistro es mayor en personas con mayor peso corporal. No es un engaño deliberado: la estimación de porciones es genuinamente difícil, y ningún diario de alimentos captura el aceite de cocina con precisión.
En el lado del gasto, las ecuaciones predictivas se compararon sistemáticamente en adultos sanos y en atletas, y existen guías de mejores prácticas para medir correctamente la tasa metabólica en reposo. Todos ellos son promedios poblacionales con márgenes de error individuales de unos pocos cientos de kilocalorías.
Disponibilidad de energía, no solo ingesta
Un concepto distinto es importante para cualquiera que entrene en serio: la disponibilidad de energía es la ingesta menos el gasto por ejercicio, relativo a la masa libre de grasa. Una disponibilidad crónicamente baja suprime la función fisiológica mucho antes de que parezca un trastorno alimentario. Un ensayo aleatorizado encontró que la baja disponibilidad de energía redujo la síntesis de proteínas miofibrilares y sarcoplasmáticas, y se han probado intervenciones de educación nutricional para abordarlo.
El sueño interactúa con todo esto: la restricción del sueño cambió la composición de la pérdida de peso en un ensayo aleatorizado, y la extensión del sueño cambió la ingesta de energía medida objetivamente [6]. Más en nuestro sitio de evidencia sobre el sueño.
¿Cambia la composición de la dieta el gasto?
Un poco, y el tamaño del efecto es genuinamente discutido. Un ensayo aleatorizado prominente examinó el gasto energético durante el mantenimiento de la pérdida de peso en una dieta baja en carbohidratos [7] y reportó una diferencia; el análisis ha sido objeto de extensos debates desde entonces. El resumen honesto es que cualquier efecto es pequeño en relación con la diferencia entre lo que las personas piensan que comen y lo que realmente comen.
