NEAT is waar het merendeel van de echte variatie tussen personen zit — friemelen, houding, spontane bewegingen. Het is ook de component die tijdens een dieet stilletjes afneemt, wat een groot deel verklaart van wat mensen ervaren als een "vertragend" metabolisme.

Dubbel gelabeld water

De referentiemethode werkt door water te drinken dat is gelabeld met stabiele isotopen van waterstof en zuurstof, waarna de verschillende uitscheidingssnelheden worden gevolgd. Het verschil daartussen weerspiegelt de productie van kooldioxide, en dus het energieverbruik, over één tot twee weken van gewoon dagelijks leven. Het is duur en onbetwistbaar.

Een systematische review van het totale energieverbruik dat op deze manier is gemeten bij volwassenen geeft het huidige beeld [2], en gepoolde analyses hebben schattingen opgeleverd van zowel het rust- als het totale energieverbruik over verschillende populaties [4][5]. Onderzoek bij kinderen met een chronische ziekte toont dezelfde methodologie toegepast waar predictieve vergelijkingen het hardst falen [1].

Een meta-analyse valideerde methoden voor voedingsbeoordeling tegen dubbel gelabeld water [3]. De bevinding is ongemakkelijk en consistent: mensen rapporteren hun inname aanzienlijk te laag, en deze onderrapportage is groter bij mensen met een hoger lichaamsgewicht. Het is geen opzettelijk bedrog — portiegroottes inschatten is werkelijk moeilijk, en geen voedingsdagboek registreert bakolie nauwkeurig.

Wat het verbruik betreft, zijn predictieve vergelijkingen systematisch vergeleken bij gezonde volwassenen en bij sporters, en er bestaan best practices voor het correct meten van het rustmetabolisme. Al deze vergelijkingen zijn populatiegemiddelden met individuele foutmarges van enkele honderden kilocalorieën.

Energiebeschikbaarheid, niet alleen inname

Een apart concept is belangrijk voor iedereen die serieus traint: energiebeschikbaarheid is inname minus sportverbruik, relatief aan vetvrije massa. Chronisch lage beschikbaarheid onderdrukt fysiologische functies ruim voordat het op een eetstoornis begint te lijken. Een gerandomiseerd onderzoek toonde aan dat lage energiebeschikbaarheid de myofibrillaire en sarcoplasmatische eiwitsynthese verminderde, en er zijn voedingseducatie-interventies getest om dit aan te pakken.

Slaap speelt hierin ook een rol — slaapbeperking veranderde de samenstelling van gewichtsverlies in een gerandomiseerd onderzoek, en verlenging van de slaap veranderde de objectief gemeten energie-inname [6]. Meer hierover op onze website over slaaponderzoek.

Verandert de samenstelling van het dieet het energieverbruik?

Een beetje, en de grootte van het effect is echt omstreden. Een prominent gerandomiseerd onderzoek onderzocht het energieverbruik tijdens het behoud van gewichtsverlies bij een koolhydraatarm dieet [7] en rapporteerde een verschil; de analyse wordt sindsdien uitgebreid bediscussieerd. De eerlijke samenvatting is dat elk effect klein is in verhouding tot het verschil tussen wat mensen denken te eten en wat ze daadwerkelijk eten.

Veelgestelde vragen

Heb ik een traag metabolisme?
Bijna zeker niet in een mate die een gewichtsprobleem verklaart. Na correctie voor lichaamssamenstelling varieert het rustmetabolisme veel minder tussen mensen dan wordt aangenomen — en de meetfout bij het bijhouden van de inname is veel groter.
Is een metabolische test in de sportschool de moeite waard?
Indirecte calorimetrie kan nauwkeurig zijn als deze onder de juiste omstandigheden wordt uitgevoerd — nuchter, uitgerust, correct gecalibreerd. De meeste commerciële tests voldoen daar niet aan.
Verhoogt vaker eten het metabolisme?
Nee. Het thermisch effect van voeding schaalt met hoeveel je eet, niet met in hoeveel maaltijden je dat verdeelt.
Waarom stagneert mijn gewichtsverlies?
