Právě v NEAT se skrývá většina skutečné variability mezi jednotlivci — nervózní pohyby, držení těla, spontánní pohyb. Je to také složka, která během diety nenápadně klesá, což je z velké části to, co lidé vnímají jako „zpomalení“ metabolismu.

Dvojitě značená voda

Referenční metoda spočívá v tom, že vypijete vodu označenou stabilními izotopy vodíku a kyslíku a následně se sleduje jejich rozdílné vylučování. Rozdíl mezi nimi odráží produkci oxidu uhličitého, a tedy energetický výdej, po dobu jednoho až dvou týdnů běžného života. Je nákladná, ale nezpochybnitelná.

Systematický přehled celkového energetického výdeje měřeného tímto způsobem u dospělých poskytuje aktuální obraz [2], a souborné analýzy přinesly odhady klidového i celkového výdeje napříč populacemi [4][5]. Práce zaměřená na děti s chronickým onemocněním ukazuje stejnou metodiku uplatněnou tam, kde predikční rovnice selhávají nejvíce [1].

Metaanalýza validovala metody hodnocení stravy oproti dvojitě značené vodě [3]. Zjištění je nepříjemné, ale konzistentní: lidé svůj příjem výrazně podhodnocují, a toto podhodnocování je větší u lidí s vyšší tělesnou hmotností. Nejde o úmyslné zkreslování — odhad velikosti porcí je skutečně obtížný a žádný stravovací deník nezachytí přesně množství použitého kuchyňského oleje.

Na straně výdeje byly predikční rovnice systematicky srovnávány u zdravých dospělých i u sportovců a existují doporučené postupy pro správné měření klidové metabolické rychlosti. Všechny tyto rovnice představují populační průměry s individuální chybou v řádu několika set kilokalorií.

Energetická dostupnost, nikoli jen příjem

Pro každého, kdo trénuje seriózně, je důležitý odlišný koncept: energetická dostupnost je příjem minus výdej při cvičení, vztažený k tukuprosté hmotě. Chronicky nízká dostupnost tlumí fyziologické funkce dávno předtím, než to začne vypadat jako porucha příjmu potravy. Randomizovaná studie zjistila, že nízká energetická dostupnost snižuje syntézu myofibrilárních i sarkoplazmatických bílkovin, a k řešení tohoto problému byly testovány intervence zaměřené na nutriční vzdělávání.

Ovlivňuje složení stravy energetický výdej?

Trochu, a velikost tohoto efektu je skutečně sporná. Významná randomizovaná studie zkoumala energetický výdej během udržování hmotnosti po hubnutí při nízkosacharidové stravě [7] a zaznamenala rozdíl; tato analýza je od té doby předmětem rozsáhlých diskusí. Upřímné shrnutí zní, že jakýkoli takový efekt je malý ve srovnání s rozdílem mezi tím, co si lidé myslí, že jedí, a tím, co skutečně jedí.
