Bruin vet en koude-blootstelling
Bruin vet is oprecht fascinerende fysiologie en een oprecht kleine bijdrage aan de hoeveelheid energie die je verbrandt.
Wat bruin vet doet
Bruin vetweefsel is boordevol mitochondriën die UCP1 tot expressie brengen, een eiwit dat de ademhalingsketen loskoppelt van de ATP-productie. De energie die anders chemisch zou zijn vastgelegd, komt in plaats daarvan vrij als warmte. Het is een doelbewuste inefficiëntie, en het evolutionaire doel ervan is het warm houden van kleine zoogdieren en menselijke zuigelingen.
Lange tijd werd gedacht dat volwassenen het verliezen. Beeldvorming toonde het tegendeel, en een meta-analyse heeft sindsdien de prevalentie en de factoren die samenhangen met een grotere hoeveelheid ervan gekwantificeerd [3]. Een translationele review beschrijft het bredere beeld [7].
Wat het activeert
Voornamelijk kou. Een systematische review van gewoonlijke blootstelling aan kou toonde aan dat dit de activiteit van bruin vetweefsel bij mensen verhoogt [1]. De effecten van inspanning op UCP1-expressie zijn gemeta-analyseerd [5], met minder indrukwekkende resultaten dan het enthousiasme over "browning" suggereert. Voedingsinterventies zijn systematisch onderzocht [4] en de effecten zijn klein.
Één bevinding verdient extra aandacht, omdat ze veel protocoladviezen onderuithaalt: door kou geïnduceerde thermogenese varieert gedurende de dag [6]. Een ijsbad om 6 uur 's ochtends en een ijsbad om 6 uur 's avonds zijn niet dezelfde stimulus, wat betekent dat de meeste gepubliceerde protocollen geen controle houden over een variabele die ertoe doet.
Blootstelling aan kou activeert bruin vet
Het rekenprobleem
Dit is waarom dit geen afslankinterventie wordt. Zelfs royale schattingen van maximaal geactiveerd bruin vet bij volwassenen komen uit op een bijdrage van enkele tientallen kilocalorieën per dag — in de orde van grootte van een koekje. Om dat te bereiken is voortdurend koudeongemak nodig, en rillende thermogenese in gewone spieren draagt bij de betrokken temperaturen meer bij dan bruin vet.
Voorbehouden bij de meting
De meeste kwantificering van bruin vet gebeurt met PET-CT met een radioactieve glucosetracer, die de glucoseopname meet in plaats van de warmteproductie en die stralingsblootstelling met zich meebrengt die herhaalde meting beperkt. Infraroodthermografie is voorgesteld als alternatief en systematisch onderzocht [2], met gemengde conclusies over de validiteit. Deze methodologische moeilijkheid is in grote mate de reden waarom dit vakgebied traag vooruitgang boekt.
