Siin on, mida numbrid tegelikult näitavad.
Väide täpsemalt
Idee on mehhanistlik ja esmapilgul usutav: süsivesikute tarbimine tõstab insuliinitaset, insuliin soodustab rasva ladestumist ja võib pärssida rasva oksüdatsiooni, mistõttu süsivesikute- ja insuliinivaesem dieet peaks teoorias jätma rohkem energiat põletamiseks, mitte ladestamiseks — isegi samal kalorikogusel. Seda nimetatakse mõnikord rasvumise süsivesiku-insuliini mudeliks. See on tõsiseltvõetav hüpotees, millel on tegelik bioloogiline usutavus, ja just seepärast väärib see otsest testimist, mitte püsivat kohta dieedifolkloori hulgas.
Mida kontrollitud uuringud näitasid
Metaanalüüs, mis analüüsis uuesti 29 kontrollitud toitmisuuringut, keskendus konkreetselt küsimusele, kas süsivesikuvaesemad dieedid suurendavad võrreldes süsivesikurikkamate dieetidega koguenergiakulu samal kalorikogusel [8]. See on selle küsimuse jaoks õige tõendusliik: kontrollitud toitmine eemaldab aruandlusvea, mis vaevab tavaelus tehtud dieediuuringuid, ja kalorite ühtlustamine eraldab koostise mõju lihtsast kalorite vähendamisest — need kaks on muidu peaaegu võimatu eraldada, probleem, mida sellel saidil käsitleb energiakulu leht toitumise jälgimise kontekstis üldisemalt.
Teine, eraldiseisev randomiseeritud uuring vaatles energiakulu konkreetselt kaalulanguse säilitamise faasis süsivesikuvaesel dieedil, mitte aktiivse langetamise ajal [17]. Säilitamisfaas on keerulisem ja asjakohasem test, kuna just see on periood, mil adaptiivne termogenees — kaalulangusele järgnev energiakulu vähenemine, mida käsitleb sellel saidil metaboolse kohanemise leht — on kõige olulisem.
Kolmas uuring vaatles otseselt puhkeoleku energiakulu pärast aktiivset kaalulangust erineva makrotoitainete koosseisuga dieetide lõikes [3].
Uuringute aus tõlgendamine
| Uuring | Mida see tegelikult eraldab | Mida see ei näita |
|---|---|---|
| 29 kontrollitud toitmisuuringu uuestianalüüs [8] | Kas süsivesikute sisaldus iseenesest, samal kalorikogusel, muudab koguenergiakulu | Kas leitud mõju on piisavalt suur, et see oleks oluline päriselulise kaalulanguse jaoks |
Kus insuliin endiselt loeb
Mitte midagi sellest ei lahenda küsimust, kas insuliiniresistentsus mõjutab kehakaalu reguleerimist laiemalt — see on eraldiseisev ja keerulisem küsimus, mida käsitleb sellel saidil insuliiniresistentsuse leht. See lahendab kitsama, konkreetse väite: kas süsivesikute vähendamine samal kalorikogusel suurendab usaldusväärselt keha põletatud kalorite hulka. Selle konkreetse väite tõenduspõhisus on nõrk. Süsivesikuvaene dieet võib paljude inimeste jaoks endiselt olla tõhus viis kaalu langetamiseks — tavaliselt seetõttu, et see muudab üldise vähem söömise lihtsamaks, mitte täiendava kalorikulu vabastamise kaudu.
Korduma kippuvad küsimused
Kas süsivesikute vähendamine samal kalorikogusel aitab rohkem kaalu langetada?
Miks tundub inimestele, et süsivesikuvaesed dieedid neile "lihtsalt toimivad"?
Kas rasvumise süsivesiku-insuliini mudel on ekslik?
