Полезни ли са мониторите за глюкоза на здравите?
CGM са трансформиращи при диабет. При хора без него те основно измерват неща, чиято значимост никой не е установил.
Какво измерва CGM
Нишка (филамент) в интерстициалната течност, семплирана през няколко минути, преобразувана в приблизителна стойност на кръвната глюкоза. От това следват две последствия. Тя изостава от кръвната глюкоза с няколко минути, което има значение при бързо променящи се стойности. И е калибрирана за диабетния диапазон, където изискванията за точност се различават от разчитането на малки колебания при човек, чиято глюкоза никога не напуска нормалния диапазон.
Какво е добре установено
При диабет случаят е ясен. Хибридните системи със затворен цикъл подобряват както гликемичните, така и психосоциалните резултати в рандомизирани изпитвания [7], а CGM се използва за директно сравняване на ефектите на лекарства върху вариабилността — например empagliflozin спрямо metformin [2]. Изпитванията с мезенхимни стволови клетки при диабет са го използвали като краен показател [5]. Това са приложенията, за които технологията е създадена.

Какво не е установено при хора без диабет
Три твърдения се правят рутинно и никое от тях няма подкрепа от клинични изпитвания:
- Че скоковете в недиабетен диапазон са вредни. Постпрандиалната глюкоза се повишава след хранене. Това е системата, работеща нормално. Никое изпитване не е показало, че отклонение в нормалния диапазон причинява вреда.
- Че изравняването на кривата подобрява здравните резултати. Никое рандомизирано изпитване при хора без диабет не е показало полза за какъвто и да е твърд краен показател от намаляването на вариабилността.
- Че личният ви отговор идентифицира храни, които са лоши за вас. Отговорът варира в зависимост от сън, стрес, предишни хранения, физическо натоварване, позицията на сензора и самия сензор. Два сензора, носени едновременно, често се разминават значително.
Какво наистина променя кривата
Някои находки са солидни и интересни. Закуска с ниско съдържание на въглехидрати води до по-нисък постпрандиален отговор в сравнение със закуска с ниско съдържание на мазнини [4] — не изненадващо, но добре доказано. Закуска с високо съдържание на протеин променя постпрандиалния профил [6]. Изпитването DASH4D изследва структурирано ефектите на диетата върху гликемичния контрол и вариабилността [3].
Най-изненадващото: рандомизирано изпитване установи, че естествената дневна светлина през работните часове подобрява гликемичния контрол [1]. Излагането на светлина е циркадна променлива, не диетична, и показва колко голяма част от регулацията на глюкозата се намира извън това, което сте изяли. Кога CGM наистина е полезен без диабет
- Преддиабет, при който следите реална клинична траектория.
- Симптоми на реактивна хипогликемия, при които корелирането на симптомите с реалните отчитания има диагностична стойност.
- Искрено любопитство, честно изразено, с реалистични очаквания за това какво могат да покажат данни от две седмици.
- Поведенческа обратна връзка, която е най-обоснованата употреба — някои хора се хранят по-добре, когато нещо ги наблюдава, и това е валидна причина, дори ако самите данни за глюкозата не носят информация.
